Our staff has taken the steps to prepare you for the first appointment by providing a full spectrum of the services we offer and the option to complete our new patient form prior to your visit. Our goal is to cover everything you need with a full and efficient assessment, but without waiting to see the doctor.

Our office is located in Rancho Penasquitos at the intersection of Carmel Mountain Road and Black Mountain Road, within Carmel Mountain Professional Park.
Driving directions from Interstate 15:
Take Highway 56 West to the Black Mountain Road exit and turn right. Proceed 0.8 mile north on Black Mountain Road. After crossing the intersection at Carmel Mountain Road, immediately turn right into the first driveway to the parking lot. Our office faces Black Mountain Road.

Take Highway 56 East to the Black Mountain Road exit and turn left (north) at the signal light. Proceed 0.8 mile north on Black Mountain Road. After crossing the intersection at Carmel Mountain Road, immediately turn right into the first driveway to the parking lot. Our office faces Black Mountain Road.
We offer patient forms online so you can complete them in the convenience of your own home or office. Fax us your printed and completed forms or you may bring them during your visit.

We accept cash, personal checks and Care Credit payment plans. Full payment is due at the time of service and before ordering any ophthalmic materials. Flexible spending accounts (FSAs) are accepted for all services, including co-payments and other non-covered expenses, prescriptive glasses, sunglasses, and contact lenses.
