If you are new to our office, please review the ensuing checklist before your appointment. Doing so helps our doctors serve you best, while also minimizing your registration time. Due to the thoroughness of our initial examination, please plan on spending approximately 75 minutes in our office.
1. Please download the Medical Records Request form if you have medical records that you would like to transfer to our office.
2. Download and complete our Patient Registration Form. To minimize your registration time, you can e-mail the completed form to us or simply bring it with you.
3. Bring any existing prescriptive eyewear and contact lenses. If you wear soft disposable contacts, bring in the boxes or blister packs which indicate your contact lens parameters.
4. As part of your comprehensive examination you will receive Optomap retinal imaging. Your are encouraged to learn about its benefits prior to your visit. Certain vision plans do not cover the $20 fee for this advanced diagnostic technology, even though our doctors STRONGLY recommend this measurement for all our patients.
