If you are new to our office, please review the ensuing checklist before your appointment. Doing so helps our doctors serve you best, while also minimizing your registration time. Due to the thoroughness of our initial examination, please plan on spending approximately 75 minutes in our office.

1. Download and complete our Patient Registration Form. To minimize your registration time, you can e-mail the completed form to us or simply bring it with you.

2. Bring any existing prescriptive eyewear and contact lenses. If you wear soft disposable contacts, bring in the boxes or blister packs which indicate your contact lens parameters.

3. If you are a vision plan member, we will ask you during registration whether you would like to undergo Optomap retinal imaging. Our doctors STRONGLY recommend that all our new patients undergo this advanced diagnostic screening. You are encouraged you to learn about its benefits prior to your appointment.
